Good Faith Estimate

Upon request, Podiatry Associates of Indiana will make best efforts to provide a “Good Faith Estimate” for services within 5 business days. The estimate does not include an estimate of patient out of pocket expenses such as deductibles, coinsurance, or non-covered services. The estimate is not a binding agreement between Podiatry Associates of Indiana and the patient. The price the provider charges the patient may vary from the “Good Faith Estimate” based on the patient’s medical needs. “The Good Faith Estimate” is only valid for 30 days.
